Responsibilities:
- Provide personal care and assistance with activities of daily living (ADLs) for hospice patients, including bathing, grooming, and dressing.
- Assist with mobility and transfers to ensure patient comfort and safety.
- Offer support with basic care tasks such as feeding, toileting, and hygiene.
- Provide companionship and emotional support to patients and their families.
- Maintain accurate patient records and communicate effectively with the healthcare team.
- Follow care plans under the direction of a registered nurse or physician.
Requirements:
- Current STNA (State Tested Nursing Assistant) certification required.
- Previous hospice or home health care experience is preferred.
- Strong interpersonal and communication skills.
- Compassionate, empathetic, and patient-focused attitude.
- Ability to work independently and as part of a team.
- Must be reliable, professional, and able to maintain confidentiality.
- Must be able to pass a background check and drug screening.
