Overview
We are seeking a dedicated and compassionate Social Worker to join our multidisciplinary healthcare team. Responsibilities
- Conduct thorough intake assessments and diagnostic evaluations to determine individual needs and develop personalized care plans.
- Provide individual, group, and family counseling utilizing evidence-based practices such as cognitive behavioral therapy, psychodynamic therapy, behavioral therapy, applied behavior analysis, and Christian counseling when appropriate.
- Implement psychotherapy techniques including motivational interviewing, trauma-informed care for PTSD patients, addiction counseling, and Alzheimer's care to support recovery and well-being.
- Coordinate with medical teams to ensure integrated patient care through accurate documentation in electronic medical records (EMR systems), adherence to HIPAA standards, and precise medical terminology usage.
- Manage crisis intervention situations effectively by providing immediate support and developing safety plans for at-risk individuals.
- Facilitate case management activities such as patient assessment, service planning, resource linkage, and follow-up to promote optimal health outcomes.
- Engage in community outreach programs to educate families and caregivers on special education needs, working with people with autism or disabilities, and geriatrics care including Alzheimer’s disease management.
- Collect data for clinical research projects related to behavioral health or social work interventions while ensuring compliance with research protocols.
- Collaborate with multidisciplinary teams across hospital or community settings to deliver comprehensive behavioral health services tailored to pediatric through geriatric populations.
- Maintain detailed medical records in accordance with legal standards such as ICD-9/10 coding for diagnoses and CPT coding for procedures; ensure accurate documentation of patient progress and treatment plans.
Skills
- Extensive knowledge of psychotherapy modalities including cognitive behavioral therapy, psychodynamic therapy, behavioral therapy, applied behavior analysis, and crisis management strategies.
- Proficiency in motivational interviewing techniques for behavior change facilitation.
- Experience working with children and adolescents in clinical or educational settings; familiarity with special education protocols is advantageous.
- Strong background in home & community care environments along with hospital experience involving patient care coordination.
- Ability to utilize EMR systems efficiently for documentation, data collection, and reporting purposes while maintaining HIPAA compliance.
- Knowledge of medical terminology related to mental health conditions such as PTSD, addiction disorders, autism spectrum disorder (ASD), Alzheimer’s disease, Geriatrics issues, ICD coding systems (ICD-9/10), and CPT coding.
- Skills in diagnostic evaluation of mental health disorders using standardized tools; experience conducting intake assessments is essential.
- Capable of providing culturally sensitive social work services that respect diverse backgrounds within a respectful framework aligned with Christian counseling principles if applicable.
- Familiarity with psychopharmacology basics relevant to behavioral health treatments; ability to collaborate effectively with prescribing clinicians is preferred.
- Demonstrated ability to manage crisis situations swiftly while maintaining professionalism under pressure; excellent communication skills are vital for effective collaboration within healthcare teams.
