Fairview Range Medical Center in Hibbing, MN, is seeking a Nurse Practitioner for our Palliative Care Department to join our multi-specialty practice. The Palliative Care Nurse Practitioner provides specialized medical care to patients with serious, chronic, or life-limiting illnesses in the home setting. The NP works collaboratively with physicians, nurses, social workers, and other interdisciplinary team members of our clinic and home care and hospice team to manage symptoms, improve quality of life, discuss goals of care, and support patients and families through complex medical decisions. The NP serves as a clinical resource for home care and hospice staff along with the medical director.

Our physicians and advanced practice providers are supported by a strong multi-specialty clinic structure, collaborating with colleagues in pediatrics, internal medicine, otolaryngology, orthopedics, general surgery, ENT, urology, and sports medicine. Consulting physicians are available in neurology, cardiology, and oncology. Our partnership with the University of Minnesota provides access to additional subspecialists for consultations and outreach.

The clinic is affiliated with Fairview Range Medical Center, the largest medical center in the region, featuring state-of-the-art diagnostic and surgical technology. Services include:

 

  • 24-hour Emergency Department with board-certified emergency medicine physicians
  • Comprehensive laboratory and diagnostic imaging
  • 24-hour respiratory therapy
  • Radiation therapy
  • Intensive Care Unit and Medical/Surgical Pediatric floor
  • Physical, occupational, and speech therapy
  • Cardiac rehabilitation
  • Specialty clinics including diabetes management and coumadin therapy

Key Responsibilities

 

Patient Care: Provide expert medical care and emotional support to patients with serious illnesses, focusing on symptom management and improving quality of life, lead goals of care discussions, facilitate advance directive and POLST discussions and support complex decision-making with patients and families.

 

Care Coordination: Collaborate with an interdisciplinary team, including physicians, nurses, and social workers, to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families. Identify hospice eligibility and make referrals when appropriate.

 

Assessment and Management: Conduct comprehensive patient assessments in the home and clinic setting, diagnose and manage acute and chronic conditions, prescribe medications and therapies/modalities i.e. home care services, provide advanced symptom management (pain, dyspnea, nausea, anxiety, delirium, etc.) and facilitate communication between patients, families, and the healthcare team, and order and interpret diagnostic tests.

 

Education: Educate patients and families about disease progression, symptom management and treatment options, and advance care planning, ensuring they are informed and supported throughout their care journey. Serve as a clinical consultant to home care and hospice staff.

 

Required Qualifications

 

  • Progressive clinical experience.
  • Master’s or Doctoral degree in Advanced Practice Nursing or Physician Assistant Studies.
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Current Advanced Practice Registered Nurse (APRN) license from the Minnesota Board of Nursing or current Physician Assistant certification and licensure in Minnesota.
  • DEA registration or eligibility for prescriptive authority
  • Valid driver's license and reliable transportation

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
