Position Summary:
Case Manager (RN or LPN) coordinates a resident’s skilled care according to physician orders and ensures communication of skilled care with the interdisciplinary team, resident and family.
Position Responsibilities of a Case Manager RN/LPN:
* Pre Admission: Conducts on-site assessments, verifies payer source, completes preadmission screening form, pharmacy and med surge quotes
* Admission: Review and initiate orders, communicate with care community personnel, greet resident and family
* Coordinate residents MDS’s and Care Conferences
* On-going communication regarding resident goals, progress and discharge planning
* Assumes responsibility for maintaining his/her own professional competence
* Oversees skilled residents care and works closely with the Office Manager
Knowledge, Skills and Abilities: Graduation from an accredited school of Nursing. Current license as a Licensed Practical Nurse with the State Board of Nursing. CPR certification recommended.
