Acts as a liaison between healthcare providers and the community to facilitate service integration, coordinate care, and strengthen relationships through public relations activities.
Key Responsibilities
- Conducts psychosocial assessments, develops and implements social work plans, and provides therapy for individuals, couples, families, and groups. Aims to prevent psychosocial dysfunction and enhance social functioning.
- Performs case management, resource referral, advocacy, and mediates to ensure appropriate patient placement and support.
- Assists in program development, evaluation, and quality improvement efforts. Participates in departmental activities, staff training, and research documentation.
- Master's degree in social work and valid licensure as a Clinical or Social Worker in Hawaii.
- Basic Life Support certification from the American Heart Association.
- Preferred: minimum one year of social work experience, healthcare background with medical diagnoses, and community agency networking skills.
