Overview :
Seeking dynamic outpatient orthopedic physical therapist to provide orthopedic PT services to adolescent through adult clientele with the M Health Fairview Rehab Services Flexible Workforce (FWF) team! The FWF team provides staffing coverage across multiple outpatient orthopedic therapy clinics in the greater Twin Cities metro area. Preferred candidate will have Mechanical Diagnosis and Therapy (MDT) training. Where MDT training and experience is limited, M Health Fairview offers mentoring and development opportunities for professional growth.
- FTE 0.6, authorized to work 48 hours per pay period.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, three post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Responsibilities / Job Description :
The Physical Therapist partners with the patient and their families while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and resolves treatment diagnosis
Develops an individualized plan of care through coordination
Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners care team to provide skilled intervention
Implements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and / or caregiver learning needs and provides appropriate education
Documentation / Billing
Uses acceptable medical terminology and follows professional practice standards.
Completes documentation in a timely manner per department standards
Stays in sync with department billing standards
Professional Expectations
Prioritizes tasks and patient care for effective time management.
Understands and follows professional licensure / certification requirements
Advises team members and / or supervises students as designated
Pursues professional development to support clinical standard processes
Quality Improvement
Crafting and implementing improvements, and participates in and supports entity / system initiatives
Qualifications : Required
Physical Therapy registration / licensure in the state of Minnesota or temporary license in MN
Current Basic Life Support (BLS) certification
Preferred
Doctorate degree in Physical Therapy
1 year of Physical Therapy practice
