An LPN Enteral Patient Education Liaison serves as a vital resource in supporting patients (transitioning from hospital to home), and caregivers who require enteral (tube) feeding at home. This role focuses on providing comprehensive education on enteral nutrition, including proper tube care, safe administration techniques, equipment use, troubleshooting common issues, and recognizing signs of complications. The liaison collaborates closely with RN Care Coordinators, dietitians, pharmacists, and home care teams to ensure individualized patient care plans are understood and followed.
Required Qualifications
- Completion of a certified LPN program
- 2 years recent experience in home care, long term care, clinic, urgent care or med-surg/acute care setting
- Licensed Practical Nurse in the State of Minnesota Upon Hire
- Basic Life Support (American Heart Assoc or Red Cross) Upon Hire
Preferred Qualifications
- Bachelor of Arts
- 2 years recent experience in home infusion therapy including direct patient care
- Previous customer service experience
