M Health Fairview is seeking a Family Medicine Physician to join our well-established primary care practice at M Health Fairview Clinic – Burnsville. This opportunity offers a collegial environment, strong specialty support, and the ability to focus on adult patients only.

Practice Details:

• 15-provider primary care practice 
• Adult patient panel only
• Average of 18 patients per day
• FTE = 34 patient contact hours per week
• Openings: 0.75 to 1.0 FTE (Full Time) - Flexible schedule
• Shared call rotation: 1:23
• As part of Fairview’s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
• Fully integrated EPIC electronic medical record
• Multidisciplinary team-based care model
• M Health Fairview Clinic – Burnsville is adjacent to Fairview Ridges Hospital.

Qualifications:

• Graduate of an accredited ACGME/AOA Family Medicine residency training program.
• Board Certified/Board Prepared with the American Board of Medical Specialties or American Osteopathic Association.
• Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges.

The posted pay range is for a 1.0 FTE. The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions with the same min/max are flat rate jobs based on 1.0FTE schedule.  Compensation plans based on production can allow for higher pay than the range posted.
