Assist in developing, implementing, supporting, and promoting health services strategies, tactics, policies, and programs that drive the delivery of quality healthcare.
Candidate utilizes advanced judgment and critical thinking skills to facilitate appropriate member healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources. This role involves intensive case management.
FUNDAMENTAL COMPONENTS:
The Case Manager/social services assistant is responsible for assisting to conduct face to face visits using comprehensive assessments.
The Case Manager/social services assistant is responsible for assisting in coordinating and collaborating care with member/authorized representative, PCP, and any other care team participant.
The Case Manager/social services assistant will attend interdisciplinary meetings and will help develop care plans based on assessments
The Case Manager/social services assistant will work with the member and care team to coordinate and assist with community resources.
The Case Manager/social services assistant is responsible for documenting accurately and timely in the electronic health record.
This position requires the Case Manager/social services assistant to use critical thinking and be able to problem solve any issues related to assigned resident.
BACKGROUND/EXPERIENCE
2 years clinical practice experience, e.g., SNF setting, hospital setting, alternative care setting such as home health or ambulatory care is preferred
2 years Case management/social services and discharge planning experience Required
LICENSES AND CERTIFICATIONS
- Licensed Vocational Nurse is preferred
