Description FAIRHOPE Hospice & Palliative Care, Home of The Pickering House, is actively recruiting full-time Social Workers to help those facing serious illnesses and end-of-life care. Our Social Workers will serve as advocates for patients and their families, assisting them in navigating through their treatment plan and finding resources to help them manage the emotional, familial, and financial strains. We are looking for individuals proficient in interpersonal communication and empathy, as they will be working to promote our hospice and palliative care philosophy during a highly stressful time. Responsibilities •Assess the psychosocial status of the patient/caregiver/family related to the Hospice and/or patient's diagnosed illness and the patient's environment to identify any areas of safety or concern. •Carry out social evaluations and plan interventions based on evaluation findings. •Counsel patient/family/caregivers as needed in relationship to stress, and other identified coping difficulties. •Discharge planning. Assist patient/caregiver/family with securing advance directives and funeral arrangements, as needed. Assist in transitioning Palliative Care patients to Hospice Care services. Appropriately report the need for Hospice to Intake/Admissions. •Participate in the interdisciplinary group (IDG) to review care plans and problem lists, exchange information, and care coordination for all patient and family services. •Triage patient calls and perform follow-up calls to admitted patients as needed. •Serve as liaison and support to public and private agencies as resources for patients and personnel. •Document electronically direct services provided and ongoing communication with patients, families, and community providers. •Remain HIPAA compliant at all times. •Physical Location: Hybrid work environment which requires traveling to patient homes, hospitals, and various healthcare settings. •Work Location: The service area covers Fairfield, Hocking, Perry, and neighboring counties. The main office is located in Lancaster, OH. Qualifications •LSW or MSW degree and current valid Social Work licensure in the state of Ohio. •A minimum of one year's experience working in healthcare is required and end-of-life care and/or hospice and palliative care experience is preferred. •Ability to analyze and implement proper action is essential. Demonstrate great communication and social interaction skills, as well as a compassionate individual. •Computer skills are required.
