SUMMARY
Works with the Interagency Transition Council (ITC) in representation of the program and the coordination of system of care. Coordinates allied and recovery support services for Transitional Aged Youth. Enhances the delivery of youth services through the ITC and staff throughout the continuum of care. Establish a system for Transitional Aged Youth in need of services and implements referral activities to meet those needs. This position requires excellent interpersonal communication skills, discretion, and tact.
JOB DUTIES
- Collaborates with relevant partners to build a support system for recovery.
- Represents the program on the Interagency Transition Council (ITC).
- Ensures Transitional Aged Youth allied and recovery support services are available and easy to access.
- Coordination with system of care (ITC) agencies in addressing challenges with TAY services.
- Recommends behavioral health free training to system of care agencies through the Youth and Young Adults Services Workforce Development Plan.
- Attends all required BHS and FNA meetings. Attends professional meetings, workshops and trainings (Staff Development Plan) as required.
- Documents activities by completing staff activity logs (SALS) on a daily basis.
- Completes all file documentation and reports in an accurate and timely manner, to include the Electronic Health Record (Care Logic).
- Maintains consumer confidentiality in accordance with 42 CFR Part 2 and HIPPA.
- Perform other duties as assigned.
NECESSARY KNOWLEDGE, SKILLS, AND ABILITY
- Familiarity with human resources policies and procedures

- Strong understanding and sensitivity to diverse cultures and lifestyles with demonstrated experience working effectively with Alaska Native populations.
- Strong understanding and verbal communication skills and strong interpersonal and leadership skills with the ability to organize and prioritize work assignments.
MINIMUM QUALIFICATIONS (Education & Experience)
- Associates degree in related field; a combination of experience, education, and training that provides the required knowledge, skills, and abilities may substitute for degree requirements.
- Experienced in case management.
- Experienced in working with Transitional Aged Youth.
- Minimum of two (2) years’ experience working with Alaska Native or American Indian populations.
PREFERRED QUALIFICATIONS (Education & Experience)
- Experienced in the field of substance use disorder treatment.
- Experienced in Transitional Aged Youth Peer.
WORKING ENVIRONMENT

The work is generally sedentary, requiring routine walking, standing, bending, and carrying of items weighing less than 25 lbs. The majority of work is performed in an office setting with a wide variety of people with differing functions, personalities, and abilities. The office is a non-smoking facility with outside smoking areas. Requires local travel between various program components of FNA which may involve exposure to extreme weather conditions. Works in an environment where protection of consumer confidentiality is essential. Personal Protective Equipment will be provided where/when needed.
SPECIFICATIONS
Supervision Received: Position receives direct supervision from the Project Manager of the Community Opioid Intervention Project; however, incumbent is expected to effectively perform day-to-day activity as denoted within job description. Work will be monitored through weekly meetings, monthly reports, and regular communication with the Director. General guidelines for operations are based on program requirements and the general policies and procedures of FNA.
Supervision Exercised: No supervision requirements for this position.
Driving Requirements: This position will require you to drive a FNA-owned vehicle for FNA business. Per FNA’s automotive insurance policy, in order to drive a FNA-owned vehicle, an individual must be at least 21 years of age. In order to drive a FNA-owned vehicle with consumers in the vehicle, an individual must be at least 25 years of age. Consumers may only be transported in FNA-owned vehicles and may not be transported by an employee in a personal vehicle.
This position requires eligibility for FNA automotive insurance as demonstrated by a current Alaska Driver’s License and a currently driving history and proof of personal liability insurance.
Position Impact: Impact at the program level is high. Program’s level of compliance with program standards is directly affected by the effective performance of duties, especially as they relate to providing Graf services to divisions and programs.
