Key Responsibilities:
Clinical Care
- Perform comprehensive evaluations to determine residents' functional status, needs, and goals.
- Develop and implement individualized treatment plans based on clinical findings and resident goals.
- Provide skilled occupational therapy interventions including ADL retraining, strengthening, balance training, fine motor skills, cognitive retraining, positioning, and functional mobility.
- Recommend and train residents on adaptive equipment, assistive devices, and compensatory techniques.
- Assess need for DME and collaborate with vendors/physicians as appropriate.
- Modify treatment plans as residents progress to ensure optimal outcomes.
Interdisciplinary Collaboration
- Participate in care plan meetings, IDT conferences, and communicate regularly with nursing, therapy, and medical staff.
- Educate residents, families, and caregivers on safe techniques, home programs, and ongoing care needs.
- Collaborate with PT, ST, and COTAs to coordinate and sequence care effectively.
Documentation
- Complete accurate, timely, and compliant documentation including evaluations, progress notes, daily treatments, discharge summaries, and billing.
- Maintain knowledge of Medicare, Medicaid, and insurance guidelines for SNF therapy services.
Compliance & Professional Standards
- Follow all facility policies, state practice acts, and federal regulatory requirements.
- Ensure infection control protocols and safety procedures are followed at all times.
- Maintain professional licensure and stay current with required continuing education.
Qualifications:
- Graduate of an accredited Occupational Therapy program.
- Current Louisiana OT license (or ability to obtain prior to start).
- Experience in a skilled nursing facility preferred, but not required.
- Strong communication, assessment, and problem-solving skills.
- Ability to work independently and as part of a team.
- Compassionate, patient, and resident-focused.
Work Environment:
- Skilled Nursing Facility setting with short-term rehab and long-term care residents.
