Job Summary
We are seeking a dedicated and compassionate Home Health Registered Nurse to join our dynamic healthcare team. Responsibilities

• Perform comprehensive patient assessments, including vital signs, physical examinations, and review of medical histories to develop individualized care plans.
• Administer medications and treatments such as IV infusions, injections, wound suturing, and specialized therapies like botulinum toxin treatments.
• Conduct skilled procedures including catheterizations, tube feedings, sterile dressing changes, and ostomy care following aseptic techniques.
• Monitor patient progress through ongoing evaluations and adjust care plans accordingly; communicate findings with physicians and interdisciplinary teams.
• Educate patients and their families on disease management, medication administration, mobility assistance, and health coaching strategies to promote independence.
• Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner while adhering to HIPAA regulations.
• Coordinate discharge planning, case management activities, and collaborate with other healthcare providers to ensure seamless continuity of care across settings.

Skills

• Proven experience in home health nursing with familiarity in medical-surgical, geriatrics, or critical care environments.
• Strong knowledge of physiology, anatomy, pharmacology, and diagnostic evaluation techniques including ICD coding (ICD-10/ICD-9).
• Proficiency with EMR/EHR systems such as Epic or Cerner for accurate documentation and patient record management.
• Skilled in performing complex procedures like IV insertions, phlebotomy, laboratory specimen collection/processing, and airway management.
• Experience working with individuals with disabilities or developmental delays; familiarity with memory care and dementia/Alzheimer’s care is highly valued.
• Ability to manage acute pain cases effectively while demonstrating excellent triage skills in urgent or emergency situations.
• Knowledge of infection control practices and sterile processing standards to ensure patient safety.
• Strong communication skills for patient education, team collaboration, and adherence to NCQA standards for quality care delivery.
