About Us:
We are seeking a dedicated and empathetic Medical LCSW to join our interdisciplinary team and support patients and families through complex medical journeys.
Medical LCSW Key Responsibilities:
- Provide psychosocial assessments and counseling to patients and families in medical settings (inpatient, outpatient, or home health).
- Collaborate with physicians, nurses, and case managers to develop and implement care plans.
- Assist with discharge planning, resource coordination, and crisis intervention.
- Advocate for patients' mental health and social needs within the healthcare system.
- Maintain accurate and timely documentation in the EHR.
Medical LCSW Qualifications:
- Master's degree in Social Work (MSW) from an accredited program.
- Current Hawaii LCSW license in good standing.
- Minimum 2 years of experience in a medical or hospital setting preferred.
- Strong knowledge of community resources, insurance, and care coordination.
- Knowledge of Medicare guidelines.
- Excellent communication, empathy, and problem-solving skills.
