We are seeking a compassionate and dedicated Mental Health Housing Supports Coordinator to join our team in Ann Arbor. This role focuses on assisting individuals with severe mental illness to find, maintain, and thrive in stable housing. The ideal candidate has experience in social work or counseling, supports clients with complex needs, and can work effectively with clients who may require high levels of support.
Key Responsibilities:
- Conduct intakes and needs assessments to evaluate clients’ mental health and housing needs.
- Develop and implement individualized housing support plans.
- Assist clients in finding and maintaining safe, stable housing, including liaising with landlords and housing agencies.
- Provide ongoing case management, including life skills coaching, crisis intervention, and support with daily living.
- Collaborate with mental health professionals, social service agencies, and community organizations.
- Maintain accurate documentation and reports related to client progress and program outcomes.
Qualifications:
- Experience working with individuals with severe mental illness.
- Degree or relevant experience in Social Work, Counseling, or related field.
- Valid driver’s license preferred; limited license considered.
- Knowledge of housing resources and community supports for individuals with mental health challenges.
- Ability to work independently and as part of a team, with empathy and professionalism.
Salary : $45,000 - $55,000
