St. Vincent de Paul Society Sacramento (SVDP) is a nonprofit organization dedicated to serving individuals and families experiencing hardship, with a particular focus on those facing poverty, housing instability, and significant life transitions. EXODUS PROJECT, a special works of SVDP, is committed to providing support, comprehensive services, and hope to individuals transitioning from incarceration back into the shared community. Our work is grounded in dignity, cultural humility, and meeting people where they are—often during moments of acute vulnerability. With offices in Solano and Sacramento Counties, we serve over 4,000 clients annually; services include counseling, mentoring, housing, employment services, micro-loans, transportation, system navigation, clothing, loaner phones, and legal services.

Job Summary

The Re-entry Specialist is both a management and direct service position, providing leadership and client support within Exodus Project’s re-entry services. Position supervises Pre-Release Care Coordinators and serves as a lead resource for Case Managers through case consultation, guidance, service coordination, training support, and complex case review.

Re-entry Specialist helps ensure that individuals referred by Sacramento County Behavioral Health are engaged prior to release from Sacramento County jails. The position conducts jail-based client meetings as needed, supports intakes and assessments, maintains required documentation, and helps coordinate continuity of care following release.

Re-entry Specialist also supports complex client cases, Enhanced Care Management activities, and community partnerships that advance the mission of Exodus Project.

This role requires sound judgment, strong communication, professional boundaries, accurate documentation, and the ability to work effectively with clients, staff, county partners, and community providers.

Required Qualifications

a. High school diploma or equivalent.

b. Valid driver’s license and ability to travel to jail facilities, community meetings, and service locations.

c. Ability to obtain and maintain clearance to enter Sacramento County jail facilities.

d. Experience providing direct services, case management, care coordination, reentry support, behavioral health support, housing navigation, peer support, or related community-based services.

e. Experience supervising, coaching, training, or leading direct-service staff.

f. Ability to work effectively with individuals who have complex needs, including justice involvement, housing instability, behavioral health concerns, substance use concerns, trauma histories, or other barriers.

g. Ability to communicate clearly and professionally in writing and verbally.

h. Ability to maintain confidentiality, professional boundaries, and accurate documentation.

i. Comfort learning and using technology, databases, and documentation systems.

Preferred Qualifications

a. Bachelor’s degree in social work, psychology, criminal justice, human services, public health, sociology, or a related field. Relevant field experience may substitute for formal education.

b. Experience supervising outreach workers, peer support staff, care coordinators, or other direct-service employees.

c. Experience providing services inside correctional facilities or coordinating jail-based reentry services.

d. Experience with Enhanced Care Management, Medi-Cal, managed care, housing navigation, reentry services, or community-based care coordination.

e. Lived experience with successful reentry, recovery, housing stability, or other relevant personal experience.

f. Familiarity with Sacramento County services, public systems, community resources, and reentry partners.

Compensation

Salary range: $75,000--$84,000, dependent on experience and education. Compensation package also includes vacation, holidays, and sick time; health, dental, vision, and life insurance; discount at our thrift store.
