Duties
- Provide hands-on assistance with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor and document patient vital signs including blood pressure, pulse, respiration, and temperature using Electronic Health Record (EHR) systems to ensure accurate medical records.
- Assist with medication administration following prescribed schedules while adhering to HIPAA regulations to maintain patient confidentiality.
- Observe patient conditions closely, noting changes in health status or behavior related to dementia, Alzheimer's disease, or other cognitive impairments; report findings promptly to supervising nurses or healthcare providers.
- Support patients with specialized care needs such as catheter care, hospice support, or memory care routines tailored for individuals with developmental disabilities or long-term care requirements.
- Prepare nutritious meals and assist with meal planning that aligns with dietary restrictions; perform light cleaning and laundry tasks to maintain a safe and hygienic environment.
- Utilize EMR (Electronic Medical Records) systems efficiently for documentation of medical records and care activities; ensure compliance with all safety protocols including CPR certification when applicable.
Skills
- Strong patient observation skills with the ability to recognize subtle changes in health or behavior.
- Experience working in assisted living facilities, nursing homes, or home health environments demonstrating adaptability across various care settings.
- Proficiency in medical terminology and documentation within EMR/EHR systems to accurately record patient information.
- Knowledge of ADLs assistance techniques including bathing, dressing, grooming, meal preparation, and mobility support.
- Familiarity with dementia care practices such as memory care strategies and Alzheimer's disease management.
- Ability to perform vital signs measurement accurately and respond appropriately to emergency situations like hospice or medical crises requiring CPR.
- Competence in handling specialized equipment such as Hoyer lifts or other mobility aids safely and effectively.
- Experience working with individuals who have developmental disabilities or disabilities requiring tailored caregiving approaches.
