Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
- Monitor and observe patients’ conditions regularly, documenting vital signs and changes in health status accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications following prescribed protocols while ensuring compliance with HIPAA regulations for patient privacy and confidentiality
- Provide specialized care for clients with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies
- Support hospice patients by delivering compassionate end-of-life care aligned with their wishes and comfort needs
- Perform catheter care, wound care, and other medical procedures as trained and authorized by healthcare professionals
- Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary
- Conduct light housekeeping tasks such as cleaning, laundry, and organizing to maintain a safe and healthy environment
- Assist with medical documentation related to patient care activities and report any concerns or incidents promptly to supervising staff
- Use medical terminology accurately when documenting patient information or communicating with healthcare teams
- Support clients during community outings or appointments to promote social engagement and independence
Skills
- Strong patient observation skills to identify changes in health status promptly
- Experience working in assisted living facilities, nursing homes, or home health environments
- Proficiency with EMR/EHR systems for accurate documentation of patient care activities
- Knowledge of assisting with ADLs, medication administration, catheter care, and dementia or Alzheimer’s care techniques
- Familiarity with hospice care principles and long-term care practices
- Ability to operate medical equipment such as Hoyer lifts and blood pressure monitors effectively
- Excellent communication skills for working collaboratively with clients, families, and healthcare professionals
- Understanding of medical terminology relevant to home health services
- CPR certification is required; additional certifications like hospice training or dementia care are a plus
- Experience working with people with developmental disabilities or disabilities in general is highly valued
- Ability to handle sensitive situations with compassion while maintaining professionalism at all times
- Capable of performing light housekeeping duties including cleaning, laundry, cooking, and maintaining a safe environment
