Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
- Monitor and observe patients’ health status, recording vital signs such as blood pressure, pulse, respiration, and temperature accurately in electronic health record (EHR) or electronic medical record (EMR) systems
- Support medication administration following proper protocols and maintaining detailed medical documentation in compliance with HIPAA regulations
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory impairments, including memory care and dementia-specific activities
- Assist with catheter care, wound care, hospice support, and other complex medical procedures as trained
- Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical guidelines
- Perform light housekeeping duties such as cleaning, laundry, and maintaining a safe environment to promote overall well-being
Skills
- Proven experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience
- Strong knowledge of medical terminology and documentation practices for accurate patient records
- Ability to perform patient observation effectively and recognize changes in condition that require escalation
- Skilled in working with individuals with developmental disabilities or disabilities requiring specialized care
- Certification in CPR and first aid is required; additional certifications such as hospice care or Alzheimer’s care are a plus
- Familiarity with EMR/EHR systems for medical documentation and patient records management
- Compassionate caregiver with excellent communication skills who thrives in a home & community care setting
- Ability to work independently or live-in employment arrangements while maintaining professionalism and confidentiality
