Job Summary
We are seeking a dedicated and compassionate Registered Nurse Home Health Case Manager to join our healthcare team. The ideal candidate will possess a strong background in nursing with experience in managed care, geriatrics, and home health settings. This role is critical in coordinating patient care, ensuring quality service delivery, and facilitating communication between patients, families, and healthcare providers.
13 Week Assignment HOME HEALTH RN
Home Health RN
**Must have at least 12 months of home health experience within the past 3 years.
OASIS experience required. HCHB experience strongly preferred.
Monday - Friday, 40 hours per week. Must meet minimum productivity expectation of 30 points per week
Yes there is a weekend rotation every 3rd weekend.
Yes they participate in call twice per month
Responsibilities
- Assess patient needs and develop individualized care plans based on comprehensive evaluations.
- Coordinate care across multiple disciplines to ensure seamless patient service and continuity of care.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Educate patients and families about health conditions, treatment options, and self-management strategies.
- Collaborate with healthcare teams to facilitate effective communication regarding patient status and care requirements.
- Advocate for patients to ensure they receive appropriate services and resources.
- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in managed care settings preferred; familiarity with geriatrics is a plus.
- Excellent patient service skills with the ability to communicate effectively with diverse populations.
- Ability to work collaboratively within a multidisciplinary team while maintaining a focus on patient-centered care.
- Strong organizational skills and attention to detail in managing multiple cases simultaneously.
