Responsibilities:
- Comprehensive Assessments:
- Conduct thorough assessments of clients’ physical, emotional, psychological, and social needs.
- Evaluate living conditions and safety factors.
- Understand the unique challenges posed by dementia and complex health issues.
- Individualized Care Plans:
- Develop personalized care plans in collaboration with the care team.
- Consider the specific requirements of each client, including cognitive impairments and behavioral changes.
- Address both short-term and long-term care needs.
- Coordination and Communication:
- Coordinate with healthcare providers, family members, and caregivers.
- Ensure seamless communication to provide holistic care.
- Advocate for clients’ preferences and needs throughout the care journey.
- Emotional Support and Crisis Intervention:
- Assist clients and their families in managing emotional concerns related to aging, dementia, and complex health conditions.
- Provide crisis intervention when necessary.
- Be empathetic and compassionate during challenging moments.
- Transitional Care Management:
- Facilitate transitions between care settings (e.g., hospital to home, home to assisted living).
- Ensure continuity of care during these transitions.
- Collaborate with other professionals (physicians, therapists, social workers).
- Education and Guidance:
- Educate families about the aging process, dementia progression, and coping strategies.
- Offer guidance on legal, financial, and insurance matters related to elder care.
- Empower families to make informed decisions.
- Advocacy and Problem-Solving:
- Advocate for clients’ rights and preferences.
- Navigate complex systems (hospital, insurance, legal, and community resources).
- Solve practical challenges related to caregiving.
Qualifications:
- Experience:
- Minimum of three years of experience as a geriatric care manager.
- Specialized background in dementia care and complex cases.
- Ability to handle diverse client needs.
- Skills:
- Excellent communication and interpersonal skills.
- Strong organizational abilities.
- Empathy and patience.
- Strong time management skills
- Proficiency in care coordination and crisis management.
- Certifications:
- LVN, RN or bachelor’s degree in a relevant field (e.g., Nursing, Social Work, Gerontology).
- Certification as a Geriatric Care Manager is highly desirable.

Compensation may vary within the salary range provided based on many factors including but not limited to location, experience, education, skills, licensure and certifications.
