This leadership role is essential in supporting accurate assessments, effective care planning, and strong interdisciplinary collaboration throughout the facility.
Key Responsibilities
- Oversee the accuracy, completeness, and timely submission of all MDS assessments and documentation
- Ensure compliance with state and federal regulations, including Medicare and PDPM requirements
- Conduct comprehensive resident assessments and interviews
- Develop, implement, and monitor individualized care plans for both short-term and long-term residents
- Lead and participate in interdisciplinary care plan meetings
- Collaborate with nursing staff, physicians, specialists, caregivers, and families to optimize resident outcomes
- Provide leadership, education, and ongoing support to nursing staff
- Conduct in-service training related to MDS, RAPS, Triggers, PDPM, and Medicare guidelines
- Maintain MDS and care plan schedules for Medicare and non-Medicare residents
- Active Florida RN license in good standing
- Minimum of 2 years of MDS Coordinator experience in a Skilled Nursing Facility
- Strong working knowledge of MDS, PDPM, Medicare guidelines, and care-planning processes
- Excellent organizational, leadership, and communication skills
