Key Responsibilities:
- Develop, implement, and maintain departmental policies in compliance with federal and state regulations
- Lead, mentor, and support the Social Services team
- Advocate for resident rights and foster a culture of dignity, compassion, and respect
- Participate in interdisciplinary care planning meetings
- Coordinate and oversee discharge planning for smooth resident transitions
- Monitor and improve the quality of social services to enhance resident outcomes
- Ensure accurate documentation and regulatory compliance
- Prepare and present Quality Assurance (QA) reports
- Serve as the facility’s Grievance Officer, addressing concerns with professionalism and empathy
- Bachelor’s Degree in Social Work (BSW) or related human services field (preferred)
- Minimum of 2 years of experience in a healthcare or long-term care setting
- Strong knowledge of federal and state long-term care regulations
- Excellent communication, leadership, and organizational skills
