Speech Therapist - Home Health - Los Angeles County, Orange, San Gabriel Valley, and San Bernardino Counties.

Qualifications:
- We want people to join our team who are qualified, caring, ethical, honest and professional.
- Current ST License in the state of CA with 1 years’ experience.
- Initiative diagnostic, preventive and rehabilitative procedures as appropriate to the patients care.
- Ability to Create a Plan of Care by evaluation and assessment of patient’s condition and maintain continuity of and or multidisciplinary services whenever needed to ensure good coordination of care.
- Highly Motivated with Passion for working with the senior population and the ability to blend healthcare and hospitality service philosophies.
- Timely Documentation according to state, federal and organizational guidelines.
- Dependable, Good communication and Organizational Skills.
- Ability to work alone and as a team member.
- Participation in Case Conference meetings, and In-Service Reviews.
Requirements:
- Active Speech Therapy License in the state of CA.
- Dependable Transportation, Current Driver’s License and auto Insurance.
- Current CPR
- Ability to perform the physical aspects of the position.
- TB testing.
