Job Overview
We are seeking a dedicated and compassionate Home Health Registered Nurse to provide exceptional patient care in a home and community setting. The ideal candidate will possess extensive clinical expertise, including experience with diverse patient populations such as geriatrics, pediatrics, and individuals with disabilities. This role involves comprehensive assessments, medication administration, wound care, and coordination of care plans to promote optimal health outcomes. The Home Health RN will utilize advanced EMR/EHR systems and adhere to HIPAA regulations while delivering personalized nursing services across various settings, including post-acute and chronic care environments.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations.
- Administer medications, injections, IV infusions, and perform venipuncture and catheter care following aseptic techniques.
- Manage complex medical conditions such as wound care, ostomy care, ventilator management, and tube feeding.
- Perform diagnostic evaluations including specimen collection/processing and laboratory testing.
- Utilize EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation of patient care and medical records.
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams.
- Educate patients and families on health coaching, medication management, disease process, and post-discharge instructions.
- Coordinate discharge planning and ensure smooth transitions between inpatient and outpatient services.
- Maintain compliance with infection control protocols and infection prevention standards per NCQA standards.
- Work with diverse populations including seniors in memory or behavioral health programs, pediatric patients requiring neonatal or Toddler care, and individuals with developmental disabilities or dementia.
- Respond to urgent needs such as acute pain management or trauma medicine in various settings including Level I/II trauma centers or emergency environments when applicable.
Qualifications
- Valid RN license with current registration in the state of practice.
- Proven experience in home health nursing; experience with post-acute care or hospice & palliative medicine is preferred.
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.
- Experience administering injections, IV insertion/infusion therapy, sterile processing procedures, and ventilator management is highly desirable.
- Ability to perform physical examinations and assessments accurately while maintaining HIPAA compliance.
- Excellent communication skills for effective patient education and interdisciplinary collaboration.
- Critical care experience including ICU or emergency room settings is advantageous; familiarity with trauma medicine or Level I trauma center protocols is a plus.
- Must be able to work independently in a home environment while adhering to safety standards; valid driver’s license required for travel between patient locations.
