What are the responsibilities and job description for the HOSPICE NURSE PRACTITIONER position at Exalting Hospice and Palliative Care?
Clinical CareConduct comprehensive initial and ongoing patient assessments in home and facility settings.Provide advanced symptom and pain management interventions consistent with hospice philosophy.Develop and update individualized plans of care in collaboration with the physician and interdisciplinary team.Evaluate changes in patient condition and initiate appropriate treatments including:Medication adjustmentsComfort-focused interventionsEmergent symptom crises managementProvide face-to-face visits as required for hospice eligibility recertifications and clinical updates (as applicable by payer and regulation).Medical Collaboration & OrdersCollaborate with hospice physicians to ensure timely and appropriate medical oversight.Write and manage medical orders and documentation consistent with NP scope of practice and hospice policy.Facilitate communication with attending physicians, specialists, and facility providers regarding patient status.Education & SupportProvide patient/family education on:Disease progressionMedication useSymptom managementHospice services and goals of careSupport families through the end-of-life process including grief-sensitive communication.Provide crisis intervention and compassionate guidance during transitions and actively dying phases.Interdisciplinary Team (IDT) ParticipationParticipate in interdisciplinary group (IDG/IDT) meetings and contribute to:Clinical updatesCare planningGoals of care discussionsDocumentation and compliance updatesCoordinate and collaborate with RN Case Managers, LVNs, social workers, chaplains, aides, and bereavement staff.Documentation & ComplianceMaintain accurate, timely documentation in the electronic medical record (EMR) following hospice regulations and agency policy.Ensure compliance with:Medicare Conditions of Participation (CoPs)Local, state, and federal regulationsAgency quality standards and documentation timelinesParticipate in quality assurance and performance improvement (QAPI) efforts as needed.Other DutiesParticipate in on-call rotation as assigned (if applicable).Provide support and mentorship to clinical staff when appropriate.Attend required training, meetings, and continuing education.QualificationsRequiredGraduate of an accredited Nurse Practitioner program (MSN or DNP)Current NP licensure in the state of practiceNational certification (AANP or ANCC)Active DEA license (or ability to obtain)Current CPR certificationMinimum 2 years of experience as an NP, preferably in:HospicePalliative careOncologyGeriatricsHome healthValid driver’s license, reliable transportation, and active auto insurancePreferredHospice and Palliative Nurse Practitioner certification (HPN/CHPN or similar)Experience with hospice documentation and EMR systemsStrong knowledge of end-of-life medication protocols and symptom managementKnowledge, Skills & AbilitiesExpertise in pain and symptom management (opioids, adjuvants, anxiety/dyspnea/agitation protocols)Strong communication skills with patients, caregivers, and clinical teamsAbility to lead and collaborate within an interdisciplinary teamExcellent critical thinking and clinical judgmentHigh level of compassion, emotional intelligence, and professionalismStrong time management and ability to work independently in the fieldPhysical RequirementsAbility to travel to patient locations throughout the service areaAbility to sit, stand, walk, and drive for extended periodsAbility to lift and carry up to 25 pounds as neededVisual and auditory ability necessary for patient assessment and documentationWork ConditionsField-based role with regular in-person patient visitsExposure to home environments and facility settingsPotential exposure to communicable diseases; requires adherence to infection control policies
