What Youll Do
- Provide comprehensive primary care to adult and geriatric populations
- Manage acute and chronic medical conditions
- Perform routine and follow-up visits in SNF and ALF settings
- Collaborate with interdisciplinary teams including nursing, therapy, and administration
- Document encounters accurately and efficiently within the EMR
- Participate in quality improvement and compliance initiatives
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA-C)
- Active Florida license
- Experience in primary care, geriatrics, SNF, or post-acute care preferred
- Strong clinical judgment and decision-making skills
- Ability to work independently while maintaining collaboration with supervising physicians
- Comfort with technology and electronic medical records
