Physical Therapist, Full Time
Position Summary: Under the vibrant leadership of our Therapy Coordinator, you'll provide essential physical therapy services to the wonderful participants of the Carolina Senior Care PACE Program. Your role will involve conducting thorough clinical evaluations and delivering personalized treatments across acute, restorative, and maintenance levels. Whether at our Day Center, in Nursing Homes, or in participants' homes, you'll make a real difference!
Essential Duties and Responsibilities:
- Conducts Assessments: Performs initial and annual physical therapy assessments, coordinating with the Interdisciplinary Team to develop and update care plans.
- Provides Treatment: Delivers physical therapy treatments, implements procedures, and confers with the Interdisciplinary Team on care issues.
- Evaluates and Recommends: Assesses participants' needs, follows up during treatment, recommends solutions, and evaluates adaptive equipment.
- Documentation and Reporting: Documents progress, maintains therapy notes, assists with data collection, and prepares reports.
- Supervision and Training: Supervises Physical Therapy Assistants, provides education and clinical supervision, and trains staff on body mechanics and equipment use.
Minimum Education:
- Physical Therapy License in North Carolina
Minimum Requirements:
- CPR Certification (Required)
- 1 Year Working with Seniors (Preferred)
Other Requirements :
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
