Role Description The Community Resource Navigator is a full-time, on-site role based in Buffalo, NY.
This position is responsible for meeting directly with pregnant and parenting individuals and families to assess needs, provide information on available services, and connect them with appropriate community resources and referrals. Day-to-day tasks include conducting intake and needs assessments, maintaining up-to-date knowledge of local agencies and programs, coordinating referrals and follow-ups, and supporting families in navigating access to diaper supplies, hygiene essentials, health services, and educational programs. The Community Resource Navigator also participates in community outreach events, maintains accurate documentation in organizational systems, and collaborates with staff and partners to strengthen referral networks and improve service coordination. The role involves working in a trauma-informed, culturally responsive manner and may require occasional evening or weekend hours for outreach activities.
Qualifications
- Experience in community outreach, case management, or social services, with the ability to build trust and rapport with diverse families.
- Strong communication and active listening skills, including the ability to explain resources clearly and advocate effectively for clients.
- Organizational and documentation skills, including experience maintaining accurate records, tracking referrals, and using basic computer and data entry tools.
- Knowledge of local community resources, health and human services, or willingness to learn and stay current on available programs and supports.
- Ability to work collaboratively with community partners and internal team members, and to participate in outreach events and group programming.
- Commitment to equity, trauma-informed practice, and culturally responsive support for families in underserved communities.
- High school diploma or equivalent required; associate or bachelor’s degree in social work, public health, human services, or a related field is preferred.
- Prior experience working with pregnant and parenting families, maternal and child health, or family support programs is highly beneficial.
