Responsibilities
- Complete psychosocial assessments and develop individualized plans of care.
- Provide emotional, psychological, and supportive counseling to patients, families, and caregivers.
- Assist patients and families with advance care planning, community resources, financial concerns, and end-of-life decision-making.
- Identify and address social determinants of health that may impact patient care and quality of life.
- Provide crisis intervention and emotional support during periods of change or loss.
- Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other members of the interdisciplinary team.
- Educate patients and families regarding hospice services, disease progression, coping strategies, and bereavement resources.
- Maintain timely, accurate documentation in the electronic medical record in accordance with agency policies and regulatory requirements.
- Participate in interdisciplinary team meetings as requested.
- Master's Degree in Social Work (MSW) from an accredited program required.
- Current Montana Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) eligible to practice independently within the hospice setting, as permitted by Montana law.
- Hospice, palliative care, home health, healthcare, or medical social work experience preferred.
- Excellent communication, counseling, and interpersonal skills.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Strong organizational, documentation, and critical thinking skills.
- Valid driver's license, reliable transportation, and current auto insurance.
