Certified Home Health Aide

Job Overview
We are seeking a compassionate and dedicated Certified Home Health Aide to join our team in providing exceptional in-home care to individuals with diverse needs. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using Hoyer lifts or other assistive devices.
• Monitor patient observations by tracking vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormal findings promptly.
• Administer medications and manage medication records in compliance with HIPAA regulations and medical documentation standards.
• Support clients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice care, and working with individuals with developmental disabilities or disabilities.
• Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately to document patient care activities and medical records.
• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients.
• Use medical terminology appropriately during documentation and communication with healthcare teams; assist with catheter care and other medical procedures as trained.
• Employ patient observation skills to recognize changes in condition or behavior; communicate effectively with family members and healthcare providers.
• Utilize equipment such as Hoyer lifts safely; assist with transfers and mobility to prevent falls or injuries.
• Provide emotional support and companionship to promote mental well-being and foster trustful relationships.

Experience

• Prior experience in home health care or assisted living environments is highly preferred.
• Demonstrated ability to work with seniors or individuals with disabilities in long-term care settings or nursing homes.
• Knowledge of medical terminology, medication administration protocols, and vital signs monitoring.
• Experience working with individuals who have Alzheimer’s disease or dementia-related conditions is advantageous.
• Familiarity with EMR/EHR systems for accurate medical documentation is required.
• Certification in CPR (Cardiopulmonary Resuscitation) is mandatory; additional certifications such as hospice care training or catheter care are a plus.
• Proven ability to handle sensitive situations with compassion while maintaining professionalism within HIPAA guidelines.
