Role Description The Direct Care Worker is a full-time, on-site role based in Bucks County, PA, providing hands-on support to clients in their homes. Day-to-day responsibilities include assisting with personal care activities such as bathing, grooming, dressing, and toileting, as well as helping with mobility, transfers, and safe use of assistive devices. The Direct Care Worker supports clients with light housekeeping, meal preparation, medication reminders, and errands to promote a safe and comfortable living environment. The role also involves monitoring clients’ well-being, documenting observations, and promptly reporting changes in condition to supervisors or appropriate care team members. Direct Care Workers are expected to build respectful relationships, follow care plans and agency policies, and uphold client confidentiality and professional ethics at all times.
