Position: Hospice Admissions Registered Nurse
Location: Portland Metro Oregon
Compensation: Starting at $52.50 depending on experience
Role Overview:
As a Hospice Admissions RN, you will:
- Conduct comprehensive assessments for new hospice patients.
- Communicate with Hospice Medical Director about hospice appropriateness
- Collaborate with interdisciplinary teams to develop individualized care plans.
- Provide education and support to patients and their families about hospice services.
- Ensure timely and accurate documentation in compliance with regulatory standards.
Qualifications:
- Current RN license in the state of Oregon.
- Minimum of 2 years of nursing experience; hospice or palliative care experience required.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
