Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. Responsibilities
- Conduct comprehensive assessments of patients' health status, including vital signs and medical history.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
- Provide education to patients and families regarding disease management, medication administration, and health maintenance.
- Assist with discharge planning to ensure a smooth transition from hospital to home care.
- Maintain accurate documentation of patient care activities in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to coordinate care for patients with complex needs, including those requiring infectious disease management or pediatric care.
- Respond promptly to patient inquiries and concerns, delivering exceptional patient service at all times.
Skills
- Strong clinical skills in nursing practice, particularly in home health settings.
- Proficient in assessing vital signs and recognizing changes in patient conditions.
- Knowledge of discharge planning processes and effective communication strategies with patients and families.
- Familiarity with HIPAA regulations to ensure patient confidentiality and data protection.
