Evergreen Woods Health and rehabilitation Center is seeking an experienced Social Services Assistant (SSA) to join our team!
The ideal candidate must possess:
Working knowledge of care plans and discharge planning services
The ability to assist potential and existing residents and patients with the Medicare and Medicaid application process.
Job Requirements
Minimum one year of Long-Term Care experience strongly preferred
Minimum one year employment experience in a social services industry
