What You’ll Do:
- Provide direct patient care (medications, treatments, monitoring)
- Develop care plans in partnership with an interdisciplinary team
- Assess patients and respond to changing needs
- Work closely with nurses, therapists, and providers to achieve the best outcomes
- Communicate with patients, families, and team members
- Maintain accurate documentation
- Active Oregon RN license
- Strong teamwork and communication skills
- Ability to thrive in a fast-paced environment
- Experience in skilled nursing, rehab, or long-term care preferred
