Director of Nursing (RN) | Skilled Nursing Facility
Evergreen Commons Rehabilitation & Nursing Center is seeking an experienced, motivated, and resident-focused Director of Nursing (DON) to lead our nursing team. As the clinical leader of our 242-bed skilled nursing facility, you will oversee nursing operations for both short-term rehabilitation and long-term care while fostering a culture of excellence, teamwork, and compassionate care.

Responsibilities
Provide leadership and oversight of all nursing services and clinical operations.
Direct, organize, and evaluate resident care 24 hours a day, 7 days a week.
Ensure compliance with all federal, state, and facility regulations.
Lead Quality Assurance and Performance Improvement (QAPI) initiatives, audits, and corrective action plans.
Maintain survey readiness and support successful Department of Health surveys.
Mentor, coach, and develop nursing leaders and staff while promoting accountability and professional growth.
Collaborate with the interdisciplinary team to ensure exceptional resident outcomes.
Oversee and partner with two Assistant Directors of Nursing (ADONs) to support daily clinical operations.
Promote a positive, resident-centered culture focused on quality care and customer service.
Qualifications
Current New York State Registered Nurse (RN) license.
Graduate of an accredited school of nursing; BSN preferred.
Minimum of three (3) years of experience in geriatric or long-term care nursing.
At least two (2) years of progressive nursing leadership or nursing administration experience.
Experience with New York State Department of Health surveys and regulatory compliance preferred.
Strong leadership, communication, organizational, and problem-solving skills.
