Role Description
This is a full-time position for a Hospice Liaison. The Hospice Liaison will be responsible for building relationships with healthcare providers, educating them on hospice services, and identifying patients who may benefit from hospice care. This role involves conducting outreach to communicate the benefits of hospice care. The Liaison will act as a key point of contact for patients, families, and the medical community, providing guidance and support throughout the process.
Qualifications
- Experience with Sales and relationship-building in a healthcare setting
- Familiarity with Home Care services and coordination of patient care
- Excellent interpersonal and communication skills, with a compassionate and empathetic approach
- Ability to collaborate effectively with healthcare professionals, patients, and families
- Valid driver’s license and reliable transportation for on-site and outreach activities
- Prior experience in hospice and end of life care prefered
