RN Hospice Admissions
Monday-Friday (11am - 8pm)
As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.
Responsibilities:
- Coordinates and oversees all aspects of new patient admissions
- Collaborates with patient care teams to ensure seamless communication
- Educates families on patient condition, needs, and care
- Performs head-to-toe physical assessments
- Timely and accurately completes documentation
Required Skills/Qualifications:
Current license as a Registered Nurse; Current driver’s license. Current CPR Certification.
Two (2) years' experience as a Registered Nurse in a clinical care setting and one_ (1) year full-time hospice._
