Estherville Community Care Center is looking for a full-time RN NOC!

Position Summary

The Registered Nurse (RN) is responsible for providing comprehensive nursing care to residents in a long-term care facility while ensuring compliance with federal, state, and facility regulations. The RN assesses residents' health conditions, develops and implements individualized care plans, administers medications and treatments, supervises nursing staff, and collaborates with interdisciplinary team members to promote the highest level of resident health, safety, and quality of life.

Essential Duties and Responsibilities

  • Assess residents upon admission and perform ongoing nursing assessments.
  • Develop, implement, evaluate, and update individualized care plans.
  • Administer medications, treatments, and intravenous therapy as ordered by physicians.
  • Monitor residents for changes in condition and promptly communicate concerns to physicians, nurse practitioners, and family members.
  • Accurately document assessments, interventions, medication administration, and resident responses in the electronic medical record (EMR).
  • Supervise and provide clinical direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other nursing personnel.
  • Ensure physician orders are accurately transcribed, implemented, and followed.
  • Participate in resident admissions, transfers, and discharges.
  • Respond appropriately to medical emergencies and initiate emergency procedures when necessary.
  • Monitor infection prevention and control practices throughout the facility.
  • Ensure compliance with all state, federal, CMS, OSHA, and facility policies and procedures.
  • Participate in quality assurance and performance improvement (QAPI) activities.
  • Assist with wound care management and other specialized nursing procedures.
  • Educate residents and families regarding health conditions, medications, treatments, and discharge planning.
  • Attend interdisciplinary care plan meetings and communicate resident progress.
  • Maintain resident confidentiality in accordance with HIPAA regulations.
  • Promote resident rights, dignity, and person-centered care.
  • Perform other duties as assigned.

Qualifications

Education

  • Graduate of an accredited Registered Nursing program.
  • Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.

Licensure

  • Current, unrestricted Registered Nurse (RN) license in the state of Iowa
  • Current CPR/BLS certification required.

Experience

  • One year of nursing experience preferred.
  • Long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred.
  • Experience with electronic medical records preferred.

  • Thorough knowledge of nursing principles and practices.
  • Strong assessment and critical thinking skills.
  • Ability to recognize and respond to changes in resident condition.
  • Excellent communication and interpersonal skills.
  • Leadership and supervisory abilities.
  • Knowledge of CMS regulations and state long-term care requirements.
  • Ability to prioritize tasks in a fast-paced environment.
  • Strong documentation and organizational skills.
  • Ability to maintain confidentiality and professional standards.

Physical Requirements

  • Ability to stand, walk, bend, reach, and lift up to 50 pounds with assistance when necessary.
  • Ability to transfer and reposition residents using proper body mechanics and lifting equipment.
  • Manual dexterity sufficient to administer medications and treatments.
  • Ability to work extended shifts, weekends, holidays, and overtime as needed.
  • Ability to respond quickly during emergency situations.

Working Conditions

  • Long-term care/skilled nursing facility environment.
  • Frequent exposure to infectious diseases and bodily fluids.
  • Use of personal protective equipment (PPE) as required.
  • May work 12-hour shifts, including weekends and holidays.
