PRN NOC LPN/RN – Long-Term Care Job Description
Position Title
PRN NOC Shift LPN/RN (Licensed Practical Nurse / Registered Nurse)
Position Summary
The PRN NOC Shift LPN/RN provides skilled nursing care to residents during overnight hours in a long-term care facility. This position is responsible for assessing residents, administering medications and treatments, monitoring changes in condition, responding to emergencies, and ensuring resident safety and comfort throughout the night shift. The nurse works collaboratively with nursing assistants, healthcare providers, residents, and families to deliver quality care in accordance with facility policies and state and federal regulations.
Schedule
- PRN (as needed)
- Night shift / NOC shift (6:00 PM – 6:00 AM)
- Weekends and holidays as needed
Essential Duties and Responsibilities
- Provide direct nursing care to residents in a long-term care setting
- Administer medications, treatments, and injections as ordered by physicians
- Monitor residents for changes in condition and report concerns appropriately
- Complete nursing assessments and documentation accurately and timely
- Respond to resident call lights and medical emergencies during overnight hours
- Supervise and support Certified Nursing Assistants (CNAs)
- Ensure compliance with infection control, safety, and resident care policies
- Maintain accurate medical records and shift reports
- Communicate effectively with residents, families, physicians, and interdisciplinary team members
- Assist with admissions, discharges, and transfers as needed
- Promote resident dignity, comfort, and quality of life
Qualifications
- Current LPN or RN license in the state of employment
- Current CPR/BLS certification preferred or required
- Previous long-term care or skilled nursing experience preferred
- Ability to work independently and make sound clinical decisions
- Strong communication and organizational skills
- Ability to handle emergency situations calmly and professionally
Physical Requirements
- Ability to stand, walk, bend, and lift throughout shift
- Ability to assist residents with mobility and transfers
- Ability to lift up to 50 pounds with assistance when necessary
