Position: Community Social Worker

Schedule: Part Time 16 hours

Pay: Starting at $30/hr. - DOQ

Here are a few of the responsibilities of a Community Social Worker:

• Support coordination of resident care and services across the community’s continuum of care
• Serve as a liaison between residents, families, physicians, and community staff to ensure clear communication and coordination of care
• Maintain accurate, current, and confidential social service documentation and resident records
• Assist residents in accessing internal and external resources
• Support Social Services in ensuring advanced directives are completed, updated, and properly maintained in resident records
• Support resident wellbeing by assisting with wellness initiatives
• Coordinate short-term and long-term stays within higher levels of care
  
  

Here are a few of the qualifications we need you to have:

• Degree in Social Work, Gerontology, or a related field
• Licensure as a Social Worker in Connecticut is preferred
• 2–3 years of experience in long-term care, transitional care, memory care, or equivalent healthcare settings preferred
• Proficiency in technology, including Microsoft Office Suite
• Strong interpersonal and communication skill
