- Support coordination of resident care and services across the community’s continuum of care
- Serve as a liaison between residents, families, physicians, and community staff to ensure clear communication and coordination of care
- Maintain accurate, current, and confidential social service documentation and resident records
- Assist residents in accessing internal and external resources
- Support Social Services in ensuring advanced directives are completed, updated, and properly maintained in resident records
- Support resident wellbeing by assisting with wellness initiatives
- Coordinate short-term and long-term stays within higher levels of care
- Degree in Social Work, Gerontology, or a related field
- Licensure as a Social Worker in Connecticut is preferred
- 2–3 years of experience in long-term care, transitional care, memory care, or equivalent healthcare settings preferred
- Proficiency in technology, including Microsoft Office Suite
- Strong interpersonal and communication skill
