REGIONAL MDS NURSEBlaire House of Worcester and West Side House are family owned and operated skilled nursing facilities that have been serving seniors for 50 years.We are seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support the MDS process at two facilities in Worcester, MA. Blaire House of Worcester is a 75-bed facility. West Side House is a 60-bed facility. Both facilities typically have a low Medicare and Managed Care census.JOB SUMMARY:Review health records including but not limited to financial and insurance information, assessments, care plans, documentation of daily care, interdisciplinary and consult notes and physician orders, medication and treatment administration records, laboratory and diagnostic reports.Conduct training with staff as needed.Participate in assessment completion process.Communicate with interdisciplinary team members on Medicare and Managed Care, Medicaid, and MDS related concerns.Gathering and sharing of clinical information for MDS process.Quality Assurance and Performance Improvement (QAPI) Involvement.EDUCATION: LPN or RN graduate from an accredited school of nursing. Current licensure with State Board of Nurse ExaminersTRAINING AND EXPERIENCE: At least one year of nursing experience in a long term care setting. Previous audit experience in area of Medicare and/or case mix helpful. Understanding of reimbursement issues and systems at Federal and State levels. Thorough knowledge of principles and methods involved in in-service instruction of nursing and auxiliary personnel.
