Our growing Health Home Division is a "Virtual Home" for Medicaid recipients with chronic health conditions. It is a care management model where all individual caregivers communicate to address patient's needs in a comprehensive manner. Essen's health home division has a team of care coordinators who function as connectors and coordinators to a patient's care team and connect members to the social services they need, including housing, transportation to appointments, medication reminders, educational courses, financial support and more.
Job Overview
We are seeking a dedicated and organized Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing patient care, ensuring effective communication between patients and healthcare providers, and facilitating a seamless experience within our medical office. This position requires a strong understanding of medical terminology and administrative support, particularly in a medical or dental office setting.
Responsibilities
- Coordinate patient care by scheduling appointments, managing patient records, and ensuring timely follow-ups.
- Serve as the primary point of contact for patients, addressing inquiries and providing information regarding services offered.
- Collaborate with healthcare professionals to develop and implement care plans tailored to individual patient needs.
- Maintain accurate documentation of patient interactions, treatment plans, and progress notes in compliance with healthcare regulations.
- Assist in the management of office operations, including inventory control and supply ordering.
- Educate patients on procedures, treatments, and health maintenance strategies to promote overall wellness.
- Ensure adherence to privacy regulations and maintain confidentiality of patient information at all times.
Qualifications
- Bachelors Degree preferred but Associates degree required.
- Previous experience in case management, Care Coordination, medical administrative support or a similar role is preferred.
- Familiarity with medical terminology is essential for effective communication within the healthcare environment.
- EMR / EHR experience preferred
- Experience working in a medical or dental office setting is highly desirable.
- Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent interpersonal skills with a compassionate approach to patient care.
- Proficiency in using office software and electronic health record (EHR) systems is an advantage.
