Job Summary We are seeking a Cardiovascular Nurse Practitioner with at least one year of outpatient cardiology experience to support a growing cardiology service line. This role works collaboratively with cardiologists and clinical staff to provide high-quality, patient-centered cardiovascular care in an outpatient setting.

Job Summary We are seeking a Cardiovascular Nurse Practitioner with at least one year of outpatient cardiology experience to support a growing cardiology service line. This role works collaboratively with cardiologists and clinical staff to provide high-quality, patient-centered cardiovascular care in an outpatient setting.
Responsibilities include
- Conduct comprehensive cardiovascular assessments, including history, physical exams, and risk stratification
- Manage and follow patients with cardiac conditions such as hypertension, heart failure, coronary artery disease, arrhythmias, and hyperlipidemia
- Order, interpret, and review diagnostic tests (EKGs, echocardiograms, stress tests, Holter monitors, labs)
- Develop and implement treatment plans in collaboration with cardiology physicians
- Prescribe medications and adjust therapies in accordance with evidence-based guidelines
- Provide patient education on disease management, lifestyle modification, and medication adherence
- Document clinical encounters accurately in the electronic medical record
- Coordinate care with referring providers, imaging, and ancillary services
- Participate in quality improvement initiatives and clinical protocols
- Active Nurse Practitioner license in NY
- Minimum of 1 year of outpatient cardiology Nurse Practitioner experience
- Board certification (ANCC or AANP)
- Active DEA (or eligibility to obtain)
- Strong knowledge of cardiovascular disease management
- Excellent communication and clinical judgment skills
- Experience in outpatient cardiology or multispecialty practices
- Familiarity with EPIC or similar EMR systems
- Experience managing high-volume patient panels
