Lifestyle Medicine Coordinator (Part-Time, 24 hrs/week) — 1-Year Role
Esperanza Health Center — Philadelphia, PA
Esperanza Health Center is seeking a well-organized, compassionate, and energetic individual to support our Lifestyle Medicine (LM) program — a hands-on, community-focused initiative that helps patients prevent, manage, and reverse chronic disease through evidence-based lifestyle medicine.
This position is ideal for someone who is passionate about health equity, food-as-medicine, faith-informed community health, and supporting patients as whole persons. A heart for frontline community service and learning, plus strong organizational and people skills, are essential. This is a one-year role (grant-funded) with strong possibility of renewal pending funding.
What You’ll Do
You will support:
✔ Patient communication, scheduling, onboarding, and follow-up
✔ Data collection (surveys, waivers, vital signs, labs)
✔ Coordination with volunteers, students, and community partners
✔ Grocery pick-up/delivery & kitchen operations
✔ Food inventory & recipe distribution
✔ Early intervention referrals resource navigation (EI/Autism eval)
✔ Translation (Spanish strongly preferred)
✔ Program evaluation documentation
Hours & Structure
• Part-time: 24 hours/week
• On-site at all 3 EHC sites; reliable vehicle required
• One-year role (grant-funded), strong possibility of renewal
• Reports to Population Health
• $21/hour

QUALIFICATIONS
Required
Passion for holistic health, wellness, and lifestyle medicine principles.
Strong organizational, administrative, and communication skills.
Creative, energetic, and able to work independently and collaboratively.
Reliable vehicle and valid driver’s license (required for grocery pick-ups and inter-site travel).
Proficiency in Microsoft Office and/or Google Workspace.
Bilingual English/Spanish with strong translation skills (verbal and written).
Preferred
Experience in community health, public health, nutrition, health coaching, program coordination, or related setting.
Familiarity with Lifestyle Medicine principles and assessments.
Experience working with volunteers, students, or community-based programs.
