Client Service Navigator

We’re looking for a compassionate, energetic Client Service Navigator to support individuals receiving services through ESI. In this role, you’ll be the go‑to guide for clients and families—helping them navigate services, build independence, and feel supported every step of the way.
What You’ll Do
- Serve as the primary contact for clients and care teams
- Support and help develop person‑centered plans
- Participate in IEP/MCO meetings and advocate for client needs
- Spend most of your day engaging directly with clients—in our programs and out in the community
- Lead our MyCommunity program to help individuals grow their independence
- Build strong, positive relationships with clients, families, and staff
What You Bring
- High School Diploma/GED required; human services degree preferred
- Experience working with people with disabilities
- Strong communication, problem‑solving, and relationship‑building skills
- Ability to stay calm, supportive, and client‑focused
- Valid driver’s license ability to pass a DOJ background check
