What are the responsibilities and job description for the Early Intervention Service Coordinator position at ESC of Eastern Ohio?
POSITION: Early Intervention Service Coordinator
DUTIES (include but not limited to the following):
1Establish and maintain positive relationships with assigned families through regular home visits and
monitoring, serving as single point of contact to assist families throughout their Early Intervention (EI)
journey beginning with initial contact through transition at age 3
2 Actively participate as a member of Mahoning County's Evidenced-Based EI Team
3Share and ensure parent's rights within EI system and complete required consents
4 Observe developmental skills of infants and toddlers; conduct screenings as appropriate
5 Gather medical, developmental and social history information to present to team
6 Coordinate eligibility and enrollment process including coordination of developmental evaluation and
assessment
7 Conduct family-directed assessment to identify family's priorities
8 Provide information about community resources and make relevant referrals
9 Coordinate and facilitate the development and review of the Individualized Family Service Plan (IFSP) that
identifies needed EI services and their funding sources
QUALIFICATIONS/REQUIREMENTS (include but not limited to the following):
1 Bachelor's Degree in Social Work, Early Childhood Education or related field
2 Eligible for Service Coordinator credential issued by the Ohio Department of Developmental Disabilities
(DoDD)
3 Possess knowledge of available community resources
4 Ability to interact in a sensitive and supportive manner with families and guardians
5 Ability to efficiently plan to meet tight deadlines
6 Ability to negotiate and manage conflict
7 Ability to use coaching strategies to support parents
8 Ability to work variable and flexible hours, including evenings
9 Ability to attend required in-service trainings as offered by DoDD
10 Evidence of a valid driver's license in state of residence, good driving record and full liability coverage on
any vehicle used for transportation
11 Excellent interviewing and documentation skills
12 Good organizational and time-management skills
13 Exceptional communication skills (verbal, written, and interpersonal) for effective interaction with internal
and external customers
14 Self-starter with ability to work independently
15 Possess computer skills in various word processing and database management programs
15 Demonstrated commitment to valuing diversity & contributing to an inclusive working environment
