Duties
- Administer prescribed medications accurately following physician orders and facility protocols
- Observe and document patient responses to medications, vital signs, and overall health status
- Assist with activities of daily living (ADLs), including hygiene, mobility, and nutrition support
- Maintain precise medical documentation using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems such as Epic or Cerner
- Monitor for adverse reactions or side effects and report findings promptly to nursing staff
- Support patients with specialized needs such as tube feeding, catheter care, or Alzheimer's/dementia care
- Ensure compliance with HIPAA regulations by safeguarding patient confidentiality and medical records
- Collaborate with nursing staff to coordinate comprehensive patient care plans in various settings including home & community care environments
- Assist in medical procedures requiring knowledge of anatomy, physiology, and medical terminology
Experience
- Prior experience working in assisted living communities, nursing homes, hospitals, or home care settings is highly preferred
- Demonstrated knowledge of medication administration procedures and medical documentation practices
- Familiarity with EMR/EHR systems like Epic or Cerner is advantageous
- Experience caring for individuals with developmental disabilities or memory-related conditions such as Alzheimer’s or dementia enhances your suitability for this role
- Background in hospice or acute care environments is a plus
- Strong understanding of anatomy, physiology, vital signs monitoring, and medical terminology is essential
- Proven ability to work compassionately with seniors and individuals with disabilities in diverse settings
