Case Manager part-time

Advanced Care Hospital of Southern New Mexico We provide critical care services to patients recovering from serious illnesses or injuries. Often our patients require additional critical care services for medically complex conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation and ventilator weaning. At our 20-bed hospital, we provide specialized treatment in the Las Cruces and surrounding areas. Our hospital is the first hospital in New Mexico to earn The Joint Commission’s Gold Seal of Approval for Respiratory Failure and are a “Center of Excellence” for all respiratory failure patients, including those who require prolonged ventilator management and weaning.

We are seeking a part-time Case Manager to join our team of passionate patient caregivers!

Required Skills:

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.

• Knowledge of clinical operations and procedures.
• Ability to establish and maintain pro-active relationships with internal interdisciplinary team members, local payer, physicians, community service organizations and health care facilities.
• Ability to maintain quality, safety, and/or infection control standards.
• Demonstrates general computer skills including: data entry, word processing, email, and record management.
• Effective organizational and time management skills.
• Effective written and verbal communication skills.
• Ability to maintain proper levels of confidentiality.
• Ability to work closely and professionally with others.

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital’s mission and “Guiding Principles” into daily practice.
